About The Position

The Community Health Worker is responsible for conducting enhanced outreach, HIV/STI/HCV testing, and evidence-based intervention programming with gay, bisexual, and other men who have sex with men (MSM) and linking them with HIV preventative treatment and/or medical care. The Community Health Worker will work toward developing the quality and effectiveness of the program activities to ensure that annual program objectives are met.

Requirements

  • High school diploma or equivalent
  • personal or professional experience with HIV/MSM communities and communities of color

Responsibilities

  • Assists with development and implementation of the annual outreach plan, which includes face-to-face and social media strategies, goals and objectives for reaching the target population
  • Promotes and administers HIV/STI/HCV testing on site and at community outreach events with a behavioral risk assessment
  • Conducts outreach according to strategies identified in the annual plan, grant workplan and the enhanced outreach model.
  • Refers participants to to PEP, PrEP, HIV/STI/HCV testing and treatment, care coordination and HIV medical care; assists participants with appointment reminders if needed; accompanies participants to appointments if needed
  • Connects participants to SDOH efforts based on identified barriers and available services to include supportive counseling and MOCHA C.A.R.E.S.
  • Provides 1:1 health education around HIV/STI prevention and treatment for gay, bisexual, transgender, and men who have sex with men who identify as persons of color throughout priority populations.
