Position Purpose: Engaging Solutions will administer care through the newly created Indiana PathWays for Aging program, which focuses on integrating care for long-term services and supports (LTSS). The Community Health Worker (CHW) is responsible for serving as the initial and main point of contact between the Company and the member. The CHW supports member access to care through the completion of face-to-face and telephonic visits to complete health screening and needs assessments, leading to referrals to community resources, processing of reports, and distribution of collateral materials.
