About The Position

The Community Health Worker (CHW) coordinates and facilitates evidence-based health education programs and services for individuals living with HIV/AIDS and other underserved populations. This role assists clients in accessing social services, nutrition programs, and health education classes such as chronic disease and diabetes self-management. The CHW also supports outreach, program development, and client engagement to promote positive health outcomes. Requirements

  • High school diploma required; CHW certification (or ability to obtain within 1 year); Bachelor's in health-related field preferred
  • 1+ year in healthcare or behavioral health
  • Experience facilitating group classes and working with underserved populations
  • Proficiency in Microsoft Office and database systems
  • Strong interpersonal, communication, and customer service skills; ability to engage clients in behavior change
  • Valid driver's license; ability to lift up to 30 lbs
  • Ability to stand or sit for extended periods
  • Must complete required training
  • In-office position with flexibility for outreach
  • Required to pass a background check through the State of Nevada

  • Bachelor's in health-related field preferred

Responsibilities

  • Confirm client eligibility and assist with scheduling, onboarding, and referrals
  • Facilitate and support health education classes (chronic disease, diabetes prevention, cooking, gardening)
  • Conduct outreach to clients and community partners; promote program visibility
  • Maintain accurate client records in electronic systems (Salesforce, RWISE)
  • Ensure compliance with HIPAA and Ryan White program requirements
  • Assist with program evaluation, reporting, and quality improvement activities
  • Collaborate with healthcare providers and community organizations
  • Perform clerical tasks and manage program resources
  • Other duties as assigned
