Northwest HealthCare, six miles west of Christian, offers 24- hour emergency care, the Sleep Disorders Center and a variety of outpatient diagnostic and imaging services. Role Purpose Community Health Worker provides preventive health improvement services by working closely with interdisciplinary care team, internal/external agencies to address social determinants of health to improve the overall health and well-being of the child and family. BJC has determined this is a safety-sensitive position. The ability to work in a constant state of alertness and in a safe manner is an essential function of this job.

Responsibilities

  • Work daily as a community liaison to identify gaps in care by assisting clients in accessing the necessary resources to remove barriers categorized as social determinants of health.
  • Provide referrals for appropriate services as needed by assisting in assessments/screenings or conducting follow-up care with patients in person, via phone, home visits and/or visits to other settings where clients are accessible.
  • Establish and maintain trusting relationships with clients on a daily basis to effectively work with an inter-disciplinary team, internal departments, and with community organizations.
  • Document and track ongoing activities, care plans, and results in an effective manner while strictly adhering to the policies and procedures in place.
  • Continuously communicate and expand knowledge of community resources, services and in the hospital or community based agencies the programs provided to reduce cultural and socio-economic barriers between the clients and institutions.
