About The Position

Baltimore Medical System (BMS) is a community-based, nonprofit organization that provides quality primary/outpatient care services to communities in the Greater Baltimore area. BMS is the largest FQHC in Maryland, and we offer a variety of comprehensive primary and behavioral health services through five (5) community health centers and eight (8) school-based health centers. The Community Outreach Worker for B'more for Healthy Babies will engage in community outreach through participation in health fairs, community events and meetings with partner organizations. The Outreach Worker will provide one-on-one education, as well as group presentations, regarding B'more for Healthy Babies themes, such as Safe Sleep, Smoke Free Homes and Access to Care. The Outreach Worker will be familiar with resources available in Southeast Baltimore and assist families in connecting to these resources. Families without access to primary care will be referred to BMS services. The Outreach Worker will develop and maintain relationships with community organizations in the Patterson Park North and East area in order to disseminate and support the work of B'more for Healthy Babies. Communication skills are required outside of the work group to facilitate work groups, resolve conflict, persuade or negotiate. Represents the company externally and provides information on a variety of programs/services. Carries out responsibilities in accordance with Baltimore Medical System Core Values, policies/ procedures and applicable civil laws. Is accountable for efficient use of resources and support of the BMS mission.

Requirements

  • High School Diploma, GED, or equivalent experience
  • Maryland Community Health Worker Certification
  • Valid Maryland State Driver's License
  • CPR Certification
  • 3 Years previous experience in a health clinic setting or other client service environment such as a community agency or a neighborhood organization is required.
  • Detailed understanding of medical practice issues related to provider schedules, referrals to specialist, insurance, etc.

Nice To Haves

  • Bilingual skills (Arabic, French, Spanish, or Swahili) preferred.
