About The Position

Community Health Worker QUALIFICATIONS        Education High school graduate or equivalent         Experience/Skills One year community health-related experience in a healthcare setting Working knowledge of medical terminology Communicates and interacts professionally with others Works cooperatively with patients and their significant others, all clinic staff, other hospital workers, director, visitors, and contracted staff Works as part of a team and independently         Required Licenses/Certifications Provides health certification at the time of employment, indicating that employee is free of any infectious process, including tuberculosis, and is in general good health. Certification must be renewed annually Permits criminal background check as required by Indiana State Department of Health Provides proof of valid driver’s license CPR certified – to be renewed as required         Working Conditions/Requirements Potential exposure to infectious substances Multiple work sites Provides own reliable transportation 1. Maintains insurance against liability for personal injury, death and property damage arising out of ownership, maintenance, and use of automobile - to be renewed as applicable    ROUTINE RESPONSIBILITIES        Behavioral Expectations Consistently complies with established Behavioral Expectations         Departmental Duties Helps individuals and families access resources including health insurance, food insecurity, transportation issues, housing insecurity, safety issues, and utility resources Facilitates communication between patient, healthcare provider, and/or social services         Documentation Provides clear and accurate documentation in the electronic medical record of any and all social determinants and resources provided to the patient Sends documentation to the referring/primary care provider          Community Work Works with community to reduce social disparities Attends community networking committee meetings to stay abreast of the most up-to-date resources in the community         Department Specific Focus Keeps organized records of work completed Maintains monthly dashboard including all pertinent information: 1. Source of referral 2. Reason for referral 3. Time required to help each patient 4. Trends of social determinants Reports monthly dashboard to the department director and to the Hospital Board of Trustees

Requirements

  • High school graduate or equivalent
  • One year community health-related experience in a healthcare setting
  • Working knowledge of medical terminology
  • Communicates and interacts professionally with others
  • Works cooperatively with patients and their significant others, all clinic staff, other hospital workers, director, visitors, and contracted staff
  • Works as part of a team and independently
