The Community Health Worker (CHW) is a trusted advocate, educator, and navigator who strengthens Prisma’s status-neutral HIV prevention and care continuum by meeting people where they are and connecting them to affirming services. This grant funded role leads linkage-to-care workflows, education initiatives, and grant-required reporting/data tracking, while engaging in robust community outreach and relationship-building. This position does not supervise staff and does not administer HIV testing. The role will coordinate with Prisma’s clinical/testing staff and program partners to support testing events through education, navigation, and warm handoffs. This position requires flexibility to support evening/weekend outreach and community events. The Community Health Worker supports the Prisma Community Care mission of providing affirming and inclusive services to promote well-being and advance health equity for diverse communities and all those seeking compassionate care - especially people of color, 2SLGBTQIA+ and Queer individuals, and those affected by HIV. What your day will look like at Prisma Community Care: Linkage to Care & Navigation (Primary Grant Deliverable) You will serve as a frontline CHW providing status-neutral navigation to Prisma services, including HIV prevention (PrEP/PEP navigation), primary care, behavioral health, and supportive services. You will conduct linkage support: scheduling appointments in the field when appropriate, sending reminder calls/texts, supporting intake paperwork, and (when requested) in-person accompaniment to reduce no-shows and barriers. You will identify and help resolve barriers to care (transportation, documentation, insurance/Medicaid pathways, stigma concerns) and coordinate with internal teams and community partners. You will maintain a welcoming and affirming presence in the community while actively generating referrals and facilitating successful connections to care. Education & Outreach Implementation You will plan and execute outreach activities in community settings (events, health fairs, community centers, schools, places of worship, local businesses, and other trusted spaces), emphasizing HIV prevention education, stigma reduction, and available Prisma services. You will assist the Clinical Programs Manager of Education & Training with community education initiatives by helping coordinate sessions, supporting facilitation logistics, and assisting with distribution/ implementation of materials and curricula. You will support outreach communications, including sharing accurate prevention messaging and community resources through social media and event-based materials. Data Tracking, Documentation & Grant Reporting You will accurately document outreach encounters, referrals, and linkage activities for program evaluation and grant reporting, using required systems and workflows to track outreach-to-care outcomes. You will provide routine updates to the Outreach and Community Engagement Manager and grant/evaluation staff, ensuring data quality, timeliness, and readiness for funder reporting. Event & Community Partnership Support (No HIV Testing Administration) You will support event planning and outreach operations, including coordinating logistics with testing/clinical teams when HIV/STI testing is offered. You will build and maintain relationships with community partners and trusted leaders to increase engagement, inbound referrals, and improve reach among priority populations. You will assist with volunteer engagement and event support as needed. You will support ongoing community programs aligned with Prisma outreach strategy (e.g., Nice Package, health campaigns) as assigned.
