About The Position

The Community Health Worker is a member of the community trained to provide education and advocacy to individuals and to the community. The worker should be someone knowledgeable of the community culture and languages spoken with a goal of providing accurate health based information geared towards reducing identified health disparities. Ensure information on community services is made available to individuals. Client outreach can occur in clinics, public venues, and clients' home, and referrals to the Community Health Worker can come from health care providers, care managers, and navigators. The plan for the Community Worker is developed in conjunction with the referral and based on evidenced based practices.

Requirements
