About The Position

Be a part of our caring community and help us put health first. The Community Health Worker will develop a holistic view of patient needs related to Social Determinants of Health, identify existing barriers to engagement with necessary resources and supports, and provide education around maintenance of chronic health conditions, as well as available options for behavioral care and social support. This role serves as a liaison between the patient and the direct care providers, assisting in navigating both internal and external systems. The Community Health Worker will initiate care planning and subsequent action steps for high-risk members, coordinating with an interdisciplinary team, and support patients' self-determination to motivate them to meet their health goals. Responsibilities also include conducting Transitions of Care for Observation stays and referring patients to necessary services and supports, which may include assistance with transportation, food insecurity, navigation of and application for benefits including Medicaid, and organizing schedules of follow-up appointments.

Requirements

  • Community Health Worker certification required in Texas
  • Minimum of 2 years of experience working in human services and navigating community-based resources
  • Ability to drive to all 4 facilities (McAllen, Brownsville, Harlingen, Edinburg) as well as patient homes

Nice To Haves

  • Bachelor's Degree in applicable discipline
  • Familiarity with state Medicaid guidelines and application processes
  • Experience working with seniors' complex needs
  • Prior experience conducting home visits and knowledge of field safety practices
  • Strongly Preferred: Bilingual in English/Spanish with the ability to speak, read and write in both languages without limitations and assistance

Responsibilities
