The Community Health Worker, Youth Opioid Response, will work primarily with transitional-age youth (TAY), ages 16-25, at risk of opioid and stimulant use disorders (SUD). The CHW will provide outreach, peer support, and harm reduction education to TAY. CHW will enroll eligible TAY into Enhanced Care Management (ECM) program, to improve their whole health through outreach and engagement activities by helping patients navigate/access community services/resources and adopt healthy behaviors. This position will serve as an integral component of the Harm Reduction program and support addiction treatment services. This position will be dynamic, providing services in schools, parks, community spaces where TAY congregate, and in clinical settings.
