About The Position
As a Community Health Worker at Carilion Clinic, you will assess patient needs and assist with providing access and navigation to inpatient, community-based, and/or clinic-based services for those identified as being at-risk of serious health conditions or high utilization of the Emergency Department based on social determinants of health. You will use person-centered and trauma-informed modalities, including motivational interviewing, to engage and identify needs. Your role will involve providing support, services, and ongoing coordination, as well as accompanying patients to various appointments to advocate for their recovery and wellness. You will also facilitate communication and advocate for patients while maintaining documentation in the electronic medical record system.
