About The Position

The Community Health Worker position in Roanoke, VA, involves assessing patient needs and assisting with access and navigation to inpatient, community-based, and/or clinic-based services for individuals identified as being at-risk of serious health conditions or high utilization of the Emergency Department. The role utilizes person-centered and trauma-informed modalities, including motivational interviewing, to engage patients and identify their needs. Responsibilities include providing support, services, and ongoing coordination, as well as accompanying patients to various appointments to advocate for their recovery and wellness. The position also requires maintaining documentation in the electronic medical record system.

Requirements

• High School diploma or equivalent required.
• Two years of general work experience.
• Experience in community outreach efforts preferred.
• Experience working with underserved populations preferred.
• Community Health Worker certification preferred.
• AHA BLS-HCP certification must be obtained by end of supervised orientation.
• Community Health Worker training must be completed upon hire and reviewed annually.
• Excellent interpersonal and communication skills.
• Reliable transportation, valid driver's license, and proof of insurance required.
• Computer word-processing and data entry skills required.
• Strong collaboration skills.
• Bilingual in Spanish preferred.
• Experience using a smartphone, email, and the internet preferred.
