About The Position
Baystate Health, a nationally recognized leader in healthcare quality and safety, is looking for a full-time Community Health Worker (CHW) to join our Community Health Center Team in Springfield, MA. The CHW will develop skills to help patients and their families navigate and access health and community services while adopting healthy behaviors. This role involves working as a care team member through an integrated approach to care management and community outreach, promoting, maintaining, and improving the health of patients and their families by providing social support and informal counseling.
