The primary purpose of the Community Health Worker (CHW) is to improve the overall health and well-being of University Hospital patients. The CHW develops trusting relationships to create linkages to community resources, physical and mental health services, and address social barriers to care that impede improvement in one’s health. The CHW is an integral member of the multi-disciplinary team that works to endorse clinical teams to the community. They provide perspectives to enhance care management strategies while efficiently navigating community resources to provide patients with access and education on the importance of preventive care screenings and services.
