About The Position

The Community Health Worker assists in providing outreach and engagement with
eligible Managed Care Plan members and supporting care management through
transportation assistance to assigned appointments and social services.

As part of the Telecare Family, Sacramento Outreach Adult Recovery (SOAR) serves
250 adults aged 18+ with serious, persistent mental health issues in an
outpatient setting. Using a tiered treatment approach, SOAR supports members in
the least restrictive environment in obtaining their hope, health and dreams.

Requirements

• High School Diploma or GED required
• Two (2) years’ experience providing mental health or intensive case management services and coordination
• Paraprofessional with or without lived mental health experience
• Experience supporting individuals with physical health challenges to navigate the physical health care system
• Valid and current driver’s license, and personal vehicle insurance with your name listed as a driver.
• Willingness to use the company vehicle to drive clients to appointments and groups, etc.
• Willingness to use personal vehicle to drive clients to appointments, clinic, etc.
• Willingness to use personal vehicle to drive to program trainings, etc.
