About The Position

Overview: This position serves as a central point of contact for pregnant and postpartum individuals, ensuring coordinated, timely, and patient-centered care across the perinatal continuum. This role focuses on improving maternal and infant health outcomes by supporting access to clinical services, addressing social determinants of health, and reducing disparities through proactive care coordination. The Maternal Care Coordinator works closely with clinical providers, community partners, and interdisciplinary teams to support individuals from pregnancy through the postpartum period. Bilingual candidates are encouraged to apply; proficiency in another language is preferred but not required. This role works with a variety of grant funded programs to bridge the gap between organizations, including health care delivery systems and community health services to: increase culturally relevant knowledge about health and wellbeing; improve underserved populations’ access to services; connecting patients to community resources; and ultimately improving health outcomes.

Requirements

  • High School Diploma or Equivalent (Required)
  • Valid Driver's License with an acceptable driving record (Required)

  • One year of community health worker experience (preferred)
