About The Position

CAN Community Health is now hiring a Community Health Worker MA Schedule: Full-Time | Day Shift | Monday-Thursday 8:00 am - 5:00 pm Friday 8:00 am - 12:00 pm Are you passionate about patient care and ready to make a difference every day? This position is responsible for the provision of patient care under a Physician or Advanced Practice Nurse Practitioner according to state regulations and will be assigned to a mobile outreach unit. Works as part of a multidisciplinary team to enhance patient quality of life. Ensures that all documentation is compliant with professional and legal standards. Requirements

  • Medical Assistant Certification issued by an accredited institution approved by the State of Florida, or a minimum of 3 years’ experience working in a health care setting
  • CPR certified.
  • Electronic Medical Records classes through SuccessEHS.
  • HIV classes through local health departments or on-line.
  • Must be able to operate a motor vehicle and have valid insurance and driver’s license.
  • Must be able to pass a Level I and Level II Background check as required.

Responsibilities

  • Greet and room patients into appropriate exam room on the mobile outreach unit.
  • Collect the medical information from patient phone calls and walk-ins.
  • Performs medical intake procedures including labs, vital signs, and history.
  • Prepares patients for examinations.
  • Prepares chart to include all diagnostic documentation as needed for patient care.
  • Assist the physician during examinations.
  • Explain medical procedures to patients.
  • Administer injections.
  • Perform Electrocardiograms.
  • Conduct tests and procedures as directed, such as EKG’ venipunctures, urine dipsticks, glucometer, etc.
  • Provides assistance in all clinical areas.
  • Assist in answering phones and scheduling appointments.
  • Update Patient files as needed.
  • Maintain patient medical records.
  • Complete all required annual trainings in a timely manner to ensure staff compliance.
