About The Position

Akido is rebuilding the healthcare experience from the ground up. Through early interventions designed around social determinants of health (SDoH), world-class care focused on chronic disease, and human-centered technology, we believe we can build a healthcare model that allows for all patients to live their fullest lives. We are a fast-growing, impact-focused, Y Combinator company created out of the University of Southern California’s D-Health Lab with the idea that empowering government, healthcare, and nonprofit services with population-based data could help usher in a new era of preventive public health. Today we are building a full stack medical network that leverages our predictive capabilities to provide a frictionless experience for both our patients and care providers.

Requirements

  • High School Diploma or equivalent.
  • Bilingual in English and Spanish strongly preferred.
  • 2+ years of experience with CalAIM populations of focus (people experiencing homelessness, adults with SMI/SUD, adults transitioning from incarceration, adults with complex medical needs).
  • Ability to work in a dynamic, outdoors environment.
  • Ability to work independently as well as part of a team.
  • Ability to prioritize multiple and competing tasks.
  • Ability to communicate effectively, including articulating one’s own relevant personal experiences.
  • Excellent oral communication skills and strong interpersonal skills.
  • Ability to use computers to document information into case management software.
  • Must have a valid driver’s license, automobile insurance, and reliable transportation.

Responsibilities

  • Manage a caseload of patients, including completing assessment forms and developing care plans.
  • Build rapport with patients to increase the likelihood of positive behavior changes.
  • Coach patients to minimize risks associated with identified common health conditions and behaviors.
  • Accompany members to medical and social services appointments.
  • Connect members to appropriate programs to address barriers to care and enhance compliance.
  • Link members to local, county, and state services, and follow up with members as an advocate.
  • Introduce systems to promote self-management and self-efficacy.
  • Document information from every encounter in designated information systems.
  • Outreach and engage with eligible patients to enroll them into the Akido ECM Program.
  • Perform other duties as assigned.
