The position will follow a hybrid schedule, with hours of 40 hours per week, day shift from 8:00 am to 4:30 pm.

Requirements

  • Equal to completion of 4 years of high school plus CHW certification and two additional specialized courses or certifications.
  • Three or more years direct experience in community-based outreach.
  • Proven ability for independent decision making and problem solving while performing effectively as part of a multi-disciplinary team.
  • Solid knowledge of community resources and effective teaching/coaching skills.
  • Must be CHW certified and have two job-related certifications including but not limited to: Alcohol and Drug, SHINE/CAC or SHIP certified, Peer Recovery Coach, Health/Life Coach, Mental Health Counselling, Behavioral Health, Integrative Behavioral Health, Licensed Addiction Counsellor, National Certified Addiction Counsellor Level I and II Certifications or Interpreter Certification(s).
  • Verifiable good driving record and reliable transportation.
  • Agree to maintain all certification requirements while in the position.
  • Massachusetts Vehicle Operators Class B license is required.
  • A clear DMV driving record is also required.
  • Must be fully vaccinated against seasonal Influenza and the COVID 19 virus or to be exempt from the requirement for medical or personal reasons by signing a statement certifying you are choosing to be exempt from vaccination once hired.

Responsibilities

  • Provide culturally appropriate health education, coaching, support, information, and outreach in a variety of settings including community-based, primary care, ambulatory, and acute care settings.
  • Identify high-risk patients utilizing effective listening and communication skills to address health and social needs.
  • Utilize critical thinking to navigate systems and facilitate the coordination of care and community resources to meet the patient’s plan of care.
  • Assist patients to practice healthy behaviors and access necessary social, medical, and behavioral health support services and programs.
  • Implement culturally tailored interventions to meet patients' short-term and long-term health and social needs.
