Participate in weekly BH pediatric triage meeting to review referred patients needing in-home therapy services. Remain familiar with family and IHT programs to complete referral paperwork and navigate referred family’s transition to IHT Participate in monthly community Systems of Care Meeting and other care collaboration meetings team recommends the CHW attend Utilize IRIS referral system to routinely document local referrals and patient participation in other agencies Stay current with local updates in community programs and newly established local resources Three days in-person and two days remote. Job Summary Summary Responsible for providing advanced health education, outreach, and care coordination services to individuals and families in the community. Areas could include exercise, mental health, medication management, nutrition, health care system navigation, substance use and other health behaviors. They work closely with healthcare providers to ensure that patients receive appropriate care and support services to improve health outcomes. Does this position require Patient Care? Yes
