About The Position

The Guidance Center (TGC) is the Community Mental Health Center and Certified Behavioral Health Clinic serving Leavenworth, Atchison, and Jefferson counties in Kansas. The Guidance Center (TGC) is currently seeking a Community Health Worker II to be the newest member of our innovative interdisciplinary team. The Community Health Worker (CHW) supports client engagement, care coordination, and access to needed health and social services. This position plays a key role in the initial intake process, early engagement, care plan follow-through, crisis triage, re-engagement of no-show Clients, and ongoing outreach to ensure clients receive timely and appropriate services. The role includes supporting quality measures, closing gaps in care, and reducing barriers to treatment. This position collaborates closely with clinical staff, nursing, care coordinators, quality teams, and community partners to promote improved health outcomes, smooth care transitions, and positive experience for clients seeking services.

Requirements

  • High school diploma or equivalent
  • Minimum of 3 years full-time experience as a Care Navigator, Community Health Worker, or comparable care coordination role.
  • Must obtain Kansas CHW certification within 6 months of employment if not already certified (required).

Nice To Haves

  • associate or bachelor’s degree in health, social services, or related field preferred.
  • Current Missouri or Kansas CHW certification
  • Experience in behavioral health, social services, or healthcare settings is strongly preferred.

Responsibilities

  • Carry a caseload of approximately 250-300 clients, adjusted for acuity.
  • Conduct post-intake outreach at scheduled intervals (e.g., 1 business day, 1 week) to support follow-through on care plans and referrals.
  • Conduct outreach to clients with gaps in care (e.g., primary care, dental, behavioral health, specialty care, preventive screenings).
  • Support reengagement for clients who miss appointments.
  • Provide reminders, education, and encouragement to support adherence to treatment.
  • Proactively identify clients who have missed or canceled appointments and initiate timely outreach to re-engage them in care.
  • Implement structured outreach workflows (e.g., same-day call/text, 48-hour follow-up, 1-week check-in) to reduce no-show rates and support appointment completion.
  • Work with front desk and clinical teams to reschedule missed appointments and address underlying barriers (transportation, childcare, work hours, anxiety, etc.).
  • Track no-show recovery efforts in the EMR to support team communication and quality improvement.
