About The Position

The Community Health Worker II/Patient Navigator improves access to timely, guideline-concordant breast cancer screening and follow-up care. This role identifies eligible patients, addresses challenges (transportation, cost, language, health literacy), coordinates appointments and reminders, leads mobile mammography events, and ensures abnormal results receive prompt diagnostic evaluation. The patient navigator partners with clinics, mammogram service providers, community organizations, and payers to advance access and increase screening completion. The Patient Navigator participates in outreach and community engagement activities, assisting with community facing initiatives, demonstrating independent actions necessary to provide competent and professional assistance to meet the needs of the team and community members. The Community Health Worker II/Patient Navigator is a highly collaborative, self-motivated, creative person that is adaptable to new processes with capacity to function independently under supervision. The Patient Navigator is knowledgeable of current Breast Cancer health trends. This is a Grant Funded position.

Requirements

  • Licensed Community Health Worker (CHW) in Texas with bachelor's degree. Most relevant degree areas include public health, Health Science, health promotion, health policy, social work-related degree, or health education.
  • Demonstrated skills in implementing community engagement/outreach or health promotion initiatives and educational materials for a diverse group of students, partners, and stakeholders
  • Demonstrated skills in computer software applications (e.g., Microsoft outlook, Teams, office 365)
  • Ability to work independently, collaboratively, and exercise sound judgment
  • Detail oriented, Problem-solving, and decision-making
  • Multitasking and time management
  • Excellent communication, both verbal and written, with strong public speaking skills
  • Demonstrated experience in effective conflict management and resolution
  • Independent use of technology, virtual meeting platforms, and social media
  • Ability to learn and adapt to new processes
  • Able to work in a team and produce quality output under tight deadlines
  • Ability to adhere to organizational objectives
  • Travel locally, regionally and nationally to community engagement and outreach events, programs, activities or conferences related to job function with cost reimbursable under grant funding
  • A minimum of two (2)-year patient navigation, and community Engagement/Outreach, Health Promotions or high-school diploma/GED certification with a minimum of five (5) years patient navigation, and community Engagement/Outreach, Health Promotions or similar experience.
  • Bilingual i.e., proficient in English and Spanish
  • Ability to maintain the security or integrity of critical infrastructure, which may include communications systems, computer networks and systems, cybersecurity systems, electrical grid, hazardous waste treatment or water treatment system.

Responsibilities

  • Coordinates and leads mobile mammography events
  • Verify eligibility of individuals and recruit appropriate candidates for breast cancer screening through TSU BCSPC and coordinates care including scheduling and navigation along with the team
  • Provide breast health barrier solutions including obtaining prior mammogram films, relevant medical records, transportation, language interpretation, financial assistance, and childcare solutions
  • Proficiently provides Spanish language interpretation to Spanish-speaking only individuals
  • Coordinates and arranges appointments and scheduling for community members requesting TSU BCSPC services
  • Communicates findings of breast care services as appropriate in an effective manner and compiles all data required for progress report
  • Interacts with peers, staff, and physicians in a supportive and respectful manner. Works collaboratively and maintains active communication with team members for financial assistance and to resolve problems that are identified during navigation
  • Answers incoming telephones, troubleshoots and directs calls and requests to appropriate individuals while evaluating, documenting, and managing their needs
  • Cultivate and maintain community relationships and network to foster program recruitment efforts
  • Prepare materials, coordinates, and participates in Breast cancer education sessions, mobile mammogram screening events, outreach events, seminars, conferences, and networking activities
  • Maintain accurate event calendar and communicate event details with the team
  • Orders and maintains supplies and outreach materials for the Breast Cancer Screening & Prevention Center Program
