About The Position

The Community Health Worker II - Care Coordinator at Massachusetts General Hospital (MGH) plays a vital role in advancing health equity and improving health outcomes for primary care patients. This position is part of the Integrated Care Management Program (iCMP) and focuses on coordinating care for high-risk patients by addressing their medical and psychosocial needs. The CHW will engage with patients, conduct assessments, and collaborate with an interdisciplinary team to connect patients to necessary resources, ultimately promoting self-efficacy and better health outcomes.

Requirements

  • High school diploma or GED required.
  • Relevant prior work experience, or an associate or bachelor's degree preferred.
  • Minimum of two years of work experience in a related field.
  • Strong interpersonal and communication skills.
  • Assessment and problem-solving skills.
  • Ability to work independently and within a multi-cultural team.
  • Demonstrated organizational and time management skills.
  • Experience working as a patient navigator or community health worker preferred.

Responsibilities

  • Engage high-risk patients identified for CHW case management through telephonic communication, in-person visits, and community outreach.
  • Conduct comprehensive high-risk assessments and utilize motivational interviewing techniques to help patients identify health goals and adhere to care plans.
  • Collaborate with the interdisciplinary primary care team to identify and complete care plan goals over a 1-3 year period.
  • Provide culturally sensitive services to patients from diverse backgrounds, utilizing medical interpretation as needed.
  • Assist patients in navigating the healthcare system, including scheduling appointments and obtaining transportation.
  • Research and connect patients to community resources related to social determinants of health (SDOH) such as food, housing, and legal services.
  • Provide health coaching to support patients in managing chronic illnesses and adopting healthy behaviors.
  • Document care within the patient's electronic medical record and communicate with the iCMP and patient care team.
