About The Position
Purpose Statement/Position Summary: The Community Health Worker, under the supervision of a Master's level administrator, provides specific services to patients and their families. Coordinates case management and social care services. Demonstrates commitment to the principles of family-centered care.
Requirements
- Experience with community resource navigation, social-care support, and/or health education.
- High school diploma/GED OR 4 years of related education and/or work experience.

- Experience with public/government benefits through work or lived experiences, preferred.
