About The Position

Under close supervision performs work of routine difficulty in providing health care education in clinics and homes; performs related work as required or assigned. This position has department specific responsibility. Assists with identifying, recruiting and training high-risk individuals to attend health care classes; maintains class records, rosters, registration and follow-up forms; teaches culturally appropriate, effective health promotion and disease prevention classes and workshops for target population; maintain a current list of community resources for patients and their families to maintain and improve their health status; participate in community health fairs and events, networks with community organizations, schools, and outside agencies to provide information on available public health resources in the community; develops, prepares and conducts community outreach and recruitment to enroll individuals in health department programs; acts as advocate and makes appropriate health/community referrals; attend regular staff meetings, trainings, and other meetings as requested; conduct complete, accurate, and timely data entry about activities and outreach; maintains HIPP compliance at all times; regular and reliable attendance is required; performs other duties as assigned.

Requirements

  • High School Diploma or GED equivalent.
  • Six (6) months of health education experience or an equivalent combination of education and experience.
  • Must possess or have the ability to obtain a fingerprint clearance card within 3 months of hire.
  • Must possess and maintain a valid Arizona driver's license.
  • Requires successful completion of a background check.
