About The Position

Housing Community Health Workers (CHWs) offer support to individuals and families enrolled in El Centro’s housing-related programs. These CHWs work 1:1 with individuals, families, and housing case managers to assist with development of client health goals and achieving housing stability. This includes patients with a positive SDOH screening indicating housing-related barriers. Responsibilities: Providing timely, responsive care coordination to assigned housing clients and patients. Build relationships with community organizations and landlords through outreach and networking opportunities. Conduct in-home health assessments with clients to identify needs. Assist clients with access to benefits, navigate healthcare, and connect with needed community resources. Document all client interactions and progress accurately using our data collection platform and the Homeless Management Information System (HMIS) to mobilize quickly and provide accurate reports and data analysis. Attend and host community health and housing-related events as needed. Advocates for clients by building trusting relationships to encourage independence, access to medical care and housing stability. Deliver rent assistance payments to landlords weekly or as needed. Create, maintain, and secure client program files. Follow all program confidentiality rules and regulations. This position can also provide HIV/STI/HCV to household members. Other duties as assigned by the program supervisor. Expectations: Relationships Build positive and trusting relationships with community partners, organizations and housing clients. Provide timely, responsive care coordination to assigned housing clients and patients. Balances the needs of caseload based upon client needs and program requirements. Able to effectively manage time to connect with and advocate for clients, locate resources, document data, etc. Delivers services with safety, respect, and effectiveness. Maintains a professional rapport with the service users, peers, and other community service providers. Participation Attend and participate in all scheduled program meetings, trainings, agency staff meetings, and special events. Be on time. Complete all assigned tasks on time. Communication Timely, professional responses to all emails, calls, and texts Communicate scheduling conflicts via email to housing case managers as soon as possible. Openly share feedback, concerns, ideas, with your direct supervisor. Scheduling Create a schedule that accounts for all responsibilities. Time off should be requested two weeks in advance via email to direct supervisor. Data Collection Collect detailed and accurate data from all client and landlord encounters as well as outreach events, when applicable. Policies Follow all program and agency policies and procedures.

Requirements

  • Community health worker certification (Active Preferred or must obtain within 90 days of employment)
  • Able to independently commute to worksites.
  • Strong relational, interpersonal, organizational, and communication skills
  • Works well in a team
  • CHW certification (qualifications as shown above)

Nice To Haves

  • Proficient in Spanish (preferred)
  • Lived experience and/or work experience in social care/social support settings (preferred)
