Overview: Candidates must reside in Ross, Pike, or Scioto Counties in Ohio. Valid Ohio driver’s license and proof of auto insurance is required by hospital policy. Must pass a motor vehicle background inspection, insurance eligibility, driving qualifications and training set forth by Nationwide Children’s Hospital and must maintain qualification of insurance guidelines. Job Description Summary: Collaborates with clinical and medical staff to provide culturally sensitive one-on-one support and guidance to patients and families with social determinants of health (SDH) needs. Job Description: Essential Functions: Links patient and family to essential support services and other needs as identified, in collaboration with clinical staff. Addresses identified SDH needs and improves access to community resources. Develops and implements health education programs and materials. Collects and reports data on program activities. Establishes and maintains close relationships with housing resources, food pantries, utility companies, government agencies, treatment facilities and other community-based organizations. Assists in conducting follow-up calls to patients and families to confirm linkage with identified community resources or services, assess potential barriers, and aids in navigating resources and services. May accompany patients and families to social service appointments. Provides appropriate documentation of patient and family interactions in EPIC. Remains informed of trends, developments, as well as state-of-the-art prevention efforts and strategies, and general information regarding the targeted community.
