About The Position
Community Health Care Workers (CHWs) at Prisma Health play a crucial role in connecting patients and their families to essential resources and care within their neighborhoods. This position is designed for individuals who are passionate about serving their community and making a difference in the lives of others. CHWs will be integral members of the care model team, focusing on enabling population health by building trust and vital relationships with residents in their assigned medical neighborhoods. After being hired, CHWs will complete the necessary training and certification to ensure they are well-equipped to perform their duties effectively. The primary responsibilities of CHWs include developing strategic partnerships within the community to enhance outreach and referrals. They will establish peer-to-peer relationships that serve as a vital link between the community and the health system. Through regular home visits, CHWs will provide guidance and support to patients, assisting them in managing their health and promoting compliance with medical appointments, screenings, treatments, and medications. They will also help patients navigate the healthcare system, facilitating assistance or interventions between care visits and advocating for patients by staying informed about community resources. In addition to direct patient support, CHWs will serve as liaisons between patients and the clinical care team, reporting back on encounters and their outcomes. They will coordinate meetings for CHWs to share best practices and collaborate with clinical liaisons to enhance the effectiveness of care delivery. This role is essential in fostering a supportive environment that empowers patients to improve their health knowledge and self-sufficiency, ultimately leading to better health outcomes in the community.
