About The Position

COMMUNITY HEALTH WORKER FOR AMBULATORY/OUTPATIENT Job responsibilities: Work collaboratively with care team of doctors, nurses, and social workers, among other specialties. Coordinate care for patients and families, including medical and social needs. Complete face to face visits, evaluate care plan progress; communicate results/coordinate next steps with the care manager. Monitor ongoing needs of the patients/families to remove barriers, ensure efficiency and help improve access to care and resources. Schedule appointments for members/families. Complete required documentation linked to the care plan. Provide updates to families regarding patient condition and progress; refer families to appropriate resources for information to answer questions and address needs. Consult with care team members to address complex issues. Supports population outreach to identify population(s) at risk and assist with closing population gaps. Job details: Location: Medical Office Building, 3440 Burnet Ave., Cincinnati OH 45229 Schedule: Day Shift/40 hours per week, Monday-Friday 8a-4:30p. Schedule hours can be flexible if we meet patient needs. No evenings, weekends or holidays; no on-call. Required qualifications: Candidates must have a high school diploma or equivalent, 2-5 years of work experience in a related job discipline Have a reliable vehicle/valid driver's license. Helpful information: Current Community Health Worker (CHW) certification in the state of Ohio is preferred but not required. Attach your resume with detailed work experience. Requirements
