Position Summary Community Health Care Workers (CHW) serve the community as a bridge between the member (community) and the healthcare system (providers) through outreach and education. By providing these services, CHW’s help Aetna members attain and maintain better health outcomes, improve relationships with healthcare providers and help both parties to become more acculturated with each other. This position is part of a new initiative in addressing members needs who are most vulnerable.

Requirements

  • 2-3 years’ experience providing clinical services
  • Flexibility in work settings like the members home or other community setting
  • Experience in serving the Medicare Populations in urban or rural environments with familiarity of resources
  • Must possess reliable transportation and be willing and able to travel up to 75% of the time, within Las Vegas, Clark County and surrounding areas.
  • Mileage is reimbursed per our company expense reimbursement policy
  • Monday- Friday 8am-5pm CST with flexibility based on business needs
  • High School Diploma or G.E.D (Required)

Nice To Haves

  • Health Care Condition experience with members with Diabetes, Hypertension, Cardiovascular Disease
  • Experience in the use of Excel and Electronic Health Records
  • Community Health Worker Certification or equivalent training and experience.
  • Experience in community/outpatient setting.
  • Bilingual in English and Spanish
  • Associates Degree in related field strongly preferred
