About The Position

Requirements

  • High school diploma or equivalent.
  • Strong understanding of community-based resources.
  • Experience as a direct care worker.
  • Familiarity with the VNS Community Resource Directory and/or 211.

Responsibilities

  • Promote positive customer relations in dealing with patients, visitors, and other employees.
  • Interact with clients to promote a full range of health services and assess their willingness to enroll in available programs.
  • Provide cultural and linguistic health education information and current resource materials to individuals and families.
  • Utilize a holistic approach considering physical, psych/social, spiritual, educational, safety, and related criteria appropriate to the age of the clients served.
  • Refer families to clinical, social, and financial support using an established algorithm to assess patient and family needs.
  • Assist patients with accessing community-based resources, including support with applications, transportation needs, and follow-up on referrals.
  • Advocate for individual and community health.
  • Enhance patients' ability to effectively communicate with healthcare providers.
  • Work with staff and providers to create care plans and communicate regarding the status of referrals and outreach efforts.
  • Maintain documentation of patients served and support provided.
  • Complete data entry to track patient outcomes, identify challenges, and recognize success.
  • Maintain up-to-date information about community-based resources for appropriate referrals.
  • Identify trends in barriers experienced by families when seeking care and provide data to team members.
