Community Health Worker (CHW) Delivery Driver
About The Position
Mountain Community Health Partnership is seeking a full time Delivery Driver for the Community Health Worker department. The Driver will ensure the delivery of essential food to eligible MCHP patients in Mitchell and Yancey Counties. They will increase engagement with vulnerable populations and community organizations that assist our most vulnerable patients. This position will require occasional weekend and before/after hour of work due to the scope of the job. Please note, this position is temporary and will only be funded for one year.
Requirements
- High School diploma or equivalent.
- Valid NC Driver's License.
- Reliable transportation.
- Proof of vehicle insurance.
- Clean driving record.

Responsibilities
- Pick up and deliver food to eligible MCHP patients.
- Maintain the privacy of patients by protecting their information and only releasing information necessary for the program.
- Provide services to patients using MCHP Company vehicles and their own personal vehicle if necessary. If a personal vehicle is used, there will be travel reimbursement.
- Conduct Social Determinants of Health (SDOH) and other screenings, making appropriate referrals to Community Health Worker staff.
