Requirements

  • High school graduate or equivalent
  • Minimum two years' experience in community education, child development, health education and/or community engagement
  • Bilingual English/Spanish
  • Valid California driver’s license, current vehicle insurance and reliable transportation with minimum two years' driving experience
  • Ability and willingness to work flexible hours including evenings and weekends
  • Demonstrated ability to communicate effectively verbally with target population and all levels personnel staff in an appropriate manner
  • Ability to work independently, take initiative, and be proactive
  • Ability to track and manage time effectively
  • Experience/familiarity with computers, Microsoft Office products (Word/Excel), business e-mail communication systems and internet search capabilities

Responsibilities

  • Perform direct outreach to target population as listed on program grant and/or as directed by supervisor.
  • Provide basic information on health topics as directed by Supervisor
  • Conduct comprehensive outreach (and/or in-reach) in local area, and refer individuals to program grant activities and/or clinic
  • Complete appropriate paperwork
  • Conduct basic assessments/screenings of target population
  • Complete data collection forms
  • Participate in health fairs and other community events
  • Track incentive cards
  • Demonstrate familiarity with VCC programs and services
  • Meet minimal requirements of program objectives
  • Database entry
  • Attend meetings, as needed
  • Enhance professional growth and development through participation in educational programs, current literature review, in-service meetings and workshops
  • Support the vision, mission and goals, and demonstrate a commitment to the values, of the organization
  • Perform other duties as directed
