Requirements

• Master's Degree in Social Work, Behavioral Health, Human Services, or related field.
• 10 or more years of relevant experience.
• Certified Application Counselor or Navigator required.
• Superb public speaking ability.
• Ability to communicate effectively in person and in writing.
• Ability to work collaboratively with community organization representatives, health center staff, and clinicians.
• Ability to effectively document and schedule in the Electronic Health Record.
• Ability to accept tasks and assignments directly from clinicians and execute with high reliability and quality.
• Ability to work occasional evenings or weekends as required.
• Ability to engage effectively at outreach events in the community.
• English proficiency required.

Responsibilities

• Collaborate with individuals and families to create and supplement care plans based on trust, healing relationships, cultural humility, and understanding of social determinants of health.
• Support and promote social assistance for existing and new patients, including health insurance, medical services, housing assistance, mental health referrals, WIC and food supplementation, job placement, legal services, domestic violence assistance, translation and interpretation of documents, application support for affordable housing and utility assistance, and other public benefits.
• Enroll adults, children, and families in health insurance - both public and private.
• Conduct outreach and enrollment activities to promote health insurance for children, adults, and families.
• Provide services in the language that is most comfortable for the patient/consumer.
• Serve as an advocate between the patient and other community providers.
• Learn and share best practices for providing services, resources, and coverage for patients/consumers.
• Represent Union Community Care in coalition meetings, community meetings, or public venues.
• Serve as social assistance case manager, meeting with families frequently to help eliminate barriers and strategize next steps for achieving their goals.
• Document referrals, social services, and patient communication in the Electronic Health Record.
• Process referrals and documents on behalf of patients to ensure timely care and services.
• Assume responsibility for complex cases referred by Community Health Resource Coordinators I and II.
• Serve as an active member of Union Community Care medical and dental teams to coordinate social assistance/social services referrals.
• Identify and develop relationships with local for- and non-profit organizations for potential referrals.
• Assume full responsibility for program planning, implementation, cross-organizational collaboration, communication, publicity, reporting, and evaluation.
• Independently present to community organizations and at public meetings.
• Perform other work-related duties as assigned.
