About The Position

Under supervision, performs paraprofessional-level health care assistance duties for clients and families. At completion of the 90-day learning period, the Community Health Representative Trainee must be able to demonstrate the responsibilities as outlined under “Additional Requirements”. Requirements

  • Graduation from high school or equivalent.
  • Knowledge of: • First aid.• Behaviors and practices to achieve and maintain good health. • Health care maintenance and prevention. • Health and social services and resources.
  • Skilled in: • Oral and written communication. • Problem solving and client interviewing. • Record-keeping. • Working with diverse populations. • Performing a number of tasks simultaneously. • Working as a part of a multidisciplinary team. • Meeting deadlines and working under time constraints. • CPR- required after hire.
  • Current and valid Washington State Driver’s License.

Responsibilities

  • Responds to inquiries by phone and in person, identifies clients’ needs, schedules client appointments as appropriate and transports clients, when necessary.
  • Collects and records measurements and vital signs; interviews clients for medical, dietary and dental history as well as other needs and/or concerns.
  • Using established protocols evaluates intake data for health risk factors and coordinates care with appropriate health care provider.
  • Provides basic health and nutrition information to clients as well as referrals to health care services and available resources.
  • Serves as liaison and advocate for client, health care and community providers.
  • Conducts periodic home visits to monitor client needs.
  • Monitors and helps to maintain eyeglasses, hearing aids, dentures and other such items to insure proper fit, function and working condition.
  • Monitors patient medications to assure proper dosages and refills prescriptions if/when necessary.
  • Maintain the privacy and confidentiality of all protected health information in accordance with the Division’s Policies and Procedures, and in compliance with state and federal regulations.
  • Accompanies clients to referral appointments and reports pertinent information to CH supervisor and/or clinical and dental providers.
  • Assist with the planning and implementation of health events i.e. health fairs, health screenings etc.
  • Operates Tribal and/or GSA vehicles, ensuring they are clean and in good working condition.
  • Other related duties as assigned.
