About The Position

The Community Health Promotion Specialist plays a crucial role in leading public health initiatives aimed at reducing chronic diseases and health inequities within the community. This position involves planning, developing, implementing, and evaluating health programs, while fostering partnerships and ensuring compliance with grant requirements. The role is focused on improving population health outcomes through community engagement and collaboration with various stakeholders.

Requirements

  • Thorough working knowledge of the social determinants of health, principles and practices of community outreach, maternal child health, program planning; the principles of population-based health promotion and disease prevention; principles and practices of coalition building and developing community partnerships; developing community outreach strategies.
  • Develop and maintain effective working relationships with other County employees, representatives of other agencies and organizations, and members of the community.
  • Effectively engage public and private organizations and members of the community.
  • Communicate effectively, both verbally and in writing, to make information, education, and persuasive presentations internally and externally.
  • Ability to use active listening as well as excellent verbal and non-verbal communication skills.
  • Interacts with individuals and groups in diverse multi-cultural and socioeconomic environments using awareness and cultural sensitivity.
  • Strong critical thinking and analytical skills.

Responsibilities

  • Oversees and implements program activities, including subgrantee activities, ensuring fidelity of process and execution in all activities.
  • Serves as subject-matter-expert for program-related and grant-related matters.
  • Builds and fosters partnerships in the community in support of program mission and work.
  • Collaborates with internal programs to advance agency priorities related to the program, its mission, and activities.
  • Assists with program planning, implementation, and evaluation; works in collaboration with program evaluators in data collection and other evaluation-related activities.
  • Completes all grant-related progress reports and documentation, as assigned.
  • Develops measurable objectives; gathers, analyzes, and interprets data; and engages in performance management and continuous quality improvement activities.
  • Facilitates and sustains community-based coalitions and supports community engagement efforts with stakeholders.
  • Coordinates with supervisor to respond to the changing public health environment through problem-solving and adapting programs as needed using change management tools and best practices.
  • May lead grant-writing processes, negotiation of budgets and implementation plans with funders, and processes to identify and secure funding to address community health gaps.
  • Organizes program data for use in grant reporting, annual reports, and documents as assigned.
  • Negotiates, navigates, and reaches consensus among differing opinions to promote health through changes to systems, policies, and practices.
  • Develops and drives comprehensive evaluation and quality improvement plans.
  • Develops messaging and communications aimed at increasing the awareness of prevention initiatives and the importance of health in priority settings.
  • Collaborates internally and externally across multiple sectors to provide stakeholders with education on evidence-based strategies and best-practices in program, policy, systems, and/or environmental changes for population health.
  • Manages provision of technical assistance leading to adoption of organizational or local policy, systems, and environmental changes that support health in prioritized community settings.
  • Oversees program activities for specific grants and initiatives as assigned, including subgrantee activities, ensuring fidelity of process and execution in all activities.
