About The Position

Live Well Springfield: Convened by the Public Health Institute of Western Massachusetts, the Live Well Springfield Coalition unites over 30 organizations dedicated to fostering and maintaining a culture of health in Springfield. The coalition's past efforts have driven progress in areas such as healthy eating, active living, the built environment, economic opportunity, and education. Currently, the coalition is focused on tackling issues related to housing, environmental justice, age- and dementia-friendly communities, climate change, health, and equity. This initiative brings together a diverse group of cross-sector stakeholders committed to collaboration. By using a policy, systems, and environmental framework, the coalition has achieved significant progress at both the local and statewide levels, ensuring the ongoing promotion of health and well-being for all in Springfield. The Community Health Planning Consultant (Consultant) is accountable for planning and development of projects for the Public Health Institute of Western MA /Partners for a Healthier Community (PHIWM), including assistance to subcontractors and community partners allied with the corporation in this these efforts. The Consultant cultivates and strengthens strategic community partnerships and alliances between local, regional, and state-level coalitions and advocacy organizations; community-based nonprofit corporations; and business, social, educational, and health entities. The Consultant is accountable for assisting in strategic thinking, program planning, and working with project evaluators to achieve the corporation's strategic goals and objectives assigned to the Consultant. In this area, the Consultant is primarily responsible for implementing strategies such as convening and technical assistance to help prioritize issues and develop community partnerships, utilizing data to execute new initiatives, review program results and communicate progress. The position's role typically involves supporting a major program/initiative with planning and coordination activities involving several program elements and streams of activity within the specified program, and in many instances, including cooperative and collaborative relationships with several community partners (faith, business, education, social sector entities, and community residents).

Requirements

  • 5 years of relevant experience in a leadership role
  • Strong interpersonal, facilitation and collaborative planning skills
  • Proven abilities to work with and within teams
  • Strong written and oral communication skills
  • A high degree of computer literacy
  • Demonstrated use of community problem-solving skills
  • Demonstrated facilitative leadership experiences in a community setting
  • Strong understanding of the public health environment (including environmental health, climate resilience, and community engagement)
  • Bachelors Degree

Nice To Haves

  • 5 years of relevant experience in a role equivalent to a Program Manager of a major public health program or related program.

Responsibilities

  • Develop programs and services that promote best and emerging practices for selected priority health area.
  • Design and implement collaborative strategies with community partners, residents, and collaborators such as social organizations, faith communities, community-based organizations and so on.
  • Convenes, builds, and supports local collaborations to support community initiative.
  • Assist in initiating and sustaining grant developed programs with community partners.
  • Implements related demonstration projects or action research including the design process for a collaborative approach and strategies for change in the health area.
  • Provides consultation to partner organizations that improve the management of program start-up processes for the project.
  • Provides facilitative leadership to the project team in action planning including steps and/or activities to address the priority areas, and implementing actions with a timeline, identifiable milestones and evaluation measures.
  • Provides facilitative leadership to fellow community leaders, and offers opportunities and/or shares experiences, perspectives and expertise on issues such as partnership development, meeting planning, facilitation, and conflict management.
  • Collaborate with evaluator to develops models for measuring program performance and results and community outcomes/impact.
  • Provides strategic thinking and forecasting on how to sustain health initiative and how to stimulate collaboration among public health and community leaders to sustain project services.
