About The Position
Community Health Partners support integrated social care for members with complex health needs (social, behavioral and physical). This role involves meeting with members in various community settings such as homes, short-term nursing facilities, shelters, and hospitals as needed, acting as an extension of the care team. The position requires caring for members by identifying and addressing their barriers to and social influences on optimal health, as well as coordinating care for a caseload of members.
