Mission City Community Network (MCCN) is a not-for-profit organization with multiple clinics operating as a Federally Qualified Health Center dedicated to serving low income and under-served families in the San Fernando Valley, San Gabriel Valley and Los Angeles and San Bernardino areas. MCCN is a full service health care provider with primary care services for children, adolescents and adults. We are currently seeking Enroller/Outreach Staff to join our team. The Enroller/Outreach Staff will be responsible for reaching out to communities to provide assistance with health insurance enrollment, screening patients according to Covered California, making follow-up calls to applicants for any missing information, accessing forms in the computer system, updating information in the practice management system, and uploading application documents to each program's computerized system. The candidate must currently be or be willing to become a Certified Enrollment Counselor to assist our patients in applying for Covered California. The role involves outreaching to individuals and families with specific healthcare needs, listening to community members in a culturally sensitive way, and connecting them with necessary services. Additionally, the Enroller/Outreach Staff will provide training and guidance on navigating the Patient Portal, accessing health records, and communicating with providers. Continuous reporting of activities to the supervisor is also required.
