Collaborate with the Community Health Manager to implement initiatives based on the Community Health Needs Report and Employee Health Risk Assessment. Requirements

  • Certified Nursing Assistant or Medical Assistant from an approved program (or equivalent)
  • At least 1 year in health, education, or community outreach preferred
  • Eligible for Community Health Worker certification within the first year
  • Valid license required; clean driving record preferred
  • Advanced bilingual proficiency in English and Spanish (verbal and written)
  • Strong interpersonal, presentation, and facilitation skills
  • Ability to work empathetically and respectfully with people of all ages, cultures, and socio‑economic backgrounds
  • Flexible schedule and adaptability to community needs

Nice To Haves

  • A.A. degree or higher preferred
  • clean driving record preferred
