About The Position

Under the guidance of the Community Health Manager, you'll deliver health and wellness education to employees, underserved populations, and the broader community - making a real difference in helping individuals adopt healthy behaviors.

Requirements

• Certified Nursing Assistant or Medical Assistant from an approved program (or equivalent).
• At least 1 year in health, education, or community outreach preferred.
• Eligible for Community Health Worker certification within the first year.
• Valid license required; clean driving record preferred.
• Advanced bilingual proficiency in English and Spanish (verbal and written).
• Strong interpersonal, presentation, and facilitation skills.
• Ability to work empathetically and respectfully with people of all ages, cultures, and socio‑economic backgrounds.
• Flexible schedule and adaptability to community needs.

Nice To Haves

• A.A. degree or higher preferred.

Responsibilities

• Facilitate health promotion and wellness education programs for employees and community members.
• Support underserved populations by providing resources and guidance to encourage healthy lifestyle changes.
• Collaborate with the Community Health Manager to implement initiatives based on the Community Health Needs Report and Employee Health Risk Assessment.
• Travel independently within the community to connect with diverse groups and deliver impactful outreach.
