About The Position

Under the general supervision of the Clinical Health Team Supervisor, provides a wide range of nursing services to promote public health. These services may be performed in clinics, homes, schools, and other community settings. The responsibilities include, but are not limited to; conduct clinical nursing services, provide health education, collaborate with community partners, investigate disease outbreaks and respond to public health threats. Must demonstrate strong technical skills, experience providing health education and ability to exercise independent nursing judgment. Nurses must be able to practice in compliance with Title X, Vaccine for Children Program, Michigan Public Health Code, CDC and other relevant minimum program requirements. This is a part-time, benefitted position working 24 hours per week out of the Public Health location at 12251 James St, Holland, MI 49424. Occasional travel to the Hudsonville and Grand Haven locations as needed.

Requirements

  • Bachelor's of Science Degree in Nursing (B.S.N.) from an accredited college or university with one year of nursing experience or R.N. degree from an accredited nursing program with one year of public health nursing experience.
  • Possession of valid State of Michigan certification as a Registered Nurse required.

  • Thorough working knowledge of statutory and regulatory requirements governing the confidentiality of patient health information, including the provisions of HIPAA.
  • Ability to interact positively with patients, co-workers, health care providers and human service practitioners and community representatives from widely diverse cultural and socio-economic backgrounds and with varying levels of interpersonal and communications skills.
  • Good organizational skills and the ability to prioritize the workload.
  • Computer proficiency and working knowledge of electronic medical records documentation standards.

Responsibilities

  • Provides public health nursing and case management services to individuals and families in clinics, homes, and community settings, by assessing health care requirements, educating and providing or coordinating follow-up health and/or social services.
  • Interviews and educates patients, performs screening tests, administers vaccines and medications, documents patient information, makes referrals and performs related patient care duties.
  • Provides disease surveillance and works with an interdisciplinary team during outbreak investigation and response.
  • Sets up clinic sites for scheduled clinic operations, ensuring that all equipment is available and operational, medications and vaccines are properly stocked, stored and have inventory control management.
  • Collects, preserves, labels, and packages specimens for laboratory analysis.
  • Provides educational presentations to health care providers and the public.
  • Ensures that all client services are provided in compliance with established standards of professional practice and ethics, and Health Department policies, procedures and quality standards.
  • Helps support an equitable, safe, diverse, and inclusive workplace.
  • Performs other related duties as assigned.
