About The Position
As part of the Care Management/Case Management department, the Community Health Navigator supports the development and execution of Blue Cross Complete of Michigan's member outreach strategy to impact health outcomes, health care costs, and member engagement. This position establishes and cultivates relationships with members, community stakeholders, and providers via phone, mail, e-mail, and personal visits. Face-to-face member visits at their home or a community organization are essential to the Community Health Navigator role.
