Requirements

• High School Diploma or GED.
• 3 years of experience in community relations, outreach, customer service, or health education.

Responsibilities

• Act as a liaison to ensure members are connected with appropriate care channels such as pharmacy, insurance, billing, and referrals.
• Conduct outreach and follow-up with patients between episodes of care to promote a seamless healthcare experience.
• Assist members with appointment attendance and coordinate transportation and pharmacy needs.
• Collect and relay information to the Care Coordination team.
