About The Position

The Arlington County Fire Department is seeking a Community Health/EMS Nurse Practitioner to join our team. The person hired for this position will report to the Fire Department Medical Director and serve/act as the Qualified Health Practitioner for the Mobile Integrated Healthcare – Community Paramedicine (MIH-CP) Program. The goals of this program, also known as “Treat-No-Transport” program, are to assess, evaluate, and provide on-scene or telehealth treatment to patients who are identified by Emergency Medical Service (EMS) providers, to prevent unnecessary transports to the emergency department and, through follow-up care, mitigate frequent calls for service. The Practitioner will 1) assess and evaluate patients to deliver patient-focused healthcare and the best medical care possible in the appropriate environment; 2) navigate the complexity of resources provided by social services in order to coordinate appropriate resources with patients; 3) collaborate with on-scene Emergency Services (EMS) providers during patient evaluation to gather clinical data to determine appropriate treatment or transport to medical facility; 4) prescribe medications and calculate dosage levels for patient care; and 5) review patient EMS medical records and available on-scene medical data to ensure medical care documents are accurate and comply with standards.

Requirements

Master's degree from an accredited Physician Assistant program
One (1) year of experience as a certified Physician Assistant
Licensure from the Virginia State Board of Medicine as a Physician Assistant
Certification as a Physician Assistant (PA-C) by NCCPA
Applicant must possess, or obtain by time of appointment, a valid driver’s license, and must provide or authorize Arlington County to obtain a copy of official driving record
Background Check: A pre-employment criminal records check will be conducted. It will include checks of the following: criminal record, driving record, education, professional licensure, and credit history. You will be required to sign a release authorizing the County to obtain your background information

Nice To Haves

Preference may be given to applicants with experience in one or more of the following:
Working as a community health nurse, social worker or in a social work environment, or in an acute inpatient psychiatric unit
Pre-hospital care (either Fire or EMS), Emergency Medicine, Urgent Care, or Critical Care
Providing services for a local government and understanding of basic Fire/EMS nomenclature, rank, operations
Image Trend charting, data analysis, and Health Insurance Portability and Accountability Act (HIPAA) training
Passion for underserved populations and bringing quality healthcare to all

Responsibilities

Act as the Qualified Health Practitioner for the Mobile Integrated Healthcare – Community Paramedicine Program with a goal to assess, evaluate and treat patients to prevent unnecessary transports to the emergency department as well as mitigate frequent calls for service
Schedule visits with recently discharged patients suffering from chronic health conditions as well as patients referred by EMS crews to evaluate patients, assess conditions, discuss resource needs, and mitigate patient care challenges. Schedule follow-up visits when needed. This could include review of inpatient hospital course, discharge instructions, pharmacy review and orders for home health services
Provide treatment in place for low-acuity patients and prescribe medications and calculate dosage levels for patient care, assessing and evaluate patients to deliver patient-focused healthcare and the best medical care possible in the appropriate environment to the citizens of Arlington County
Navigate the medical system for patients with low health literacy who may rely on 911 for their chronic medical needs
Provide in person support to high utilizers of EMS system in conjunction with existing treatment protocols to increase access and support safely remaining in the community
Work closely with Arlington County Community Service Board entities like the Crisis Intervention Center to coordinate services for patients the QHP has interacted with
Conduct QAQI on QHP patient encounters to ensure patients’ needs are met and conduct ongoing assessments and evaluations of patients’ conditions
Provide CME training to EMS field providers regarding program successes or failures for ongoing program improvements
