About The Position

Summary of essential functions The Director of Community Health provides strategic, operational, and clinical leadership to advance United Regional Health Care System’s community health, population health, and chronic disease management efforts across the continuum of care. This role ensures alignment between organizational strategy, inpatient and outpatient services, and community health initiatives to improve outcomes, reduce avoidable utilization, and deliver destination, world class healthcare. The Director oversees multiple inpatient and outpatient service lines, provides administrative oversight of providers in collaboration with Medical Directors, and leads initiatives focused on care coordination, access, and community health improvement.

Requirements

Bachelor’s degree in Nursing, Public Health, Healthcare Administration, or a related field
Master’s degree in Nursing, Public Health, Healthcare Administration, Business Administration, or a related field required or must be obtained within two (2) years of hire.
Minimum of 5 years of progressive leadership experience.
Strong systems thinking with ability to design and operationalize cross‑continuum models of care
Proven leadership, communication, and team‑building skills across multidisciplinary teams
Experience with quality improvement, performance measurement, and continuous improvement
Knowledge of public health principles and factors influencing health outcomes and equity
Financial and operational acumen, including budget oversight and resource optimization
Experience leveraging health information systems and data to support clinical and operational performance
Ability to build trust and alignment across clinical, operational, and community stakeholders
Must be able to communicate effectively in English, both verbally and in writing

Nice To Haves

Active licensure in a healthcare discipline (e.g., Registered Nurse, Respiratory Therapist, or other allied health profession) is preferred.

Responsibilities

Leads the development and execution of community and population health strategies aligned with organizational priorities and community needs
Translates Community Health Needs Assessment (CHNA) findings into prioritized, measurable initiatives that address identified opportunities and gaps
Champions innovative models of care and best practices to position United Regional among the nation’s highest‑performing healthcare organizations
Ensures high‑quality, efficient operations and standardizes policies, procedures, and clinical protocols to support evidence‑based practice and regulatory compliance
Drives and is accountable for performance outcomes related to access, utilization, quality, patient and workforce experience, and community health improvement
Partners with Medical Staff Services, Human Resources, and Physician Group leadership to ensure timely and compliant provider credentialing, privileging, onboarding, and compensation processes.
Leads risk‑stratified approaches to managing patient needs, with emphasis on high‑risk and rising‑risk populations
Monitors performance data, quality metrics, and dashboards; leads corrective action and continuous improvement efforts
Ensures effective care transitions and coordination across inpatient, outpatient, and community settings
Aligns closely with community‑based providers, practices, and leadership to support coordinated care delivery
Optimizes use of Epic tools, virtual care, and digital health solutions to support care coordination and population health management.
Provides financial oversight for assigned service lines, including budgeting and resource optimization.
In collaboration with the Director of the Foundation, oversees the Community Care Fund (philanthropic), ensuring compliance with all applicable regulatory and reporting requirements and that funds are stewarded in alignment with organizational priorities, community needs, and donor intent.
Leads and participates in key committees and multidisciplinary initiatives.
Maintains current knowledge of best practices, regulatory requirements, and emerging trends related to community health, population health, and chronic disease management.
Builds and sustains collaborative relationships across the organization and with internal and external partners to advance community health priorities.
Is responsible for employee relations, corrective action, and annual performance evaluations for all staff and providers reporting to the assigned service areas.
