About The Position

Elder Options is seeking a Community Health Care Coach, to provide community health education, follow up support and coaching to patients. Services include Care Transition services supporting hospital to home transitions and/or other community-based health and wellness services. The interventions are geared toward patient self-management of chronic diseases via teaching/modeling, self-advocacy, and education with the purpose of reducing re-hospitalizations, providing individualized assessments, care planning, referrals and supports to adults with health and wellbeing goals to support their independence in the community. Home visits are required and remote work is limited.

Requirements

• Bachelor’s degree (B.A./B.S.) from four-year college or university or equivalent experience in providing human services.
• Successful completion in the Take Charge Care Transition Model is required for maintaining position.

Nice To Haves

• Experience working with older and/or disabled adults and Spanish-speaking preferred.
