About The Position

The Community Health Associate at Atlantic Health System plays a vital role in connecting patients with health and social services, addressing social determinants of health, and empowering individuals to achieve their health goals. This position involves direct outreach, documentation, and collaboration with a multidisciplinary team to enhance patient care and improve health outcomes in the community.

Requirements

  • High School diploma or GED required; higher educational degree is a plus.
  • Familiarity with the community in the service area and knowledge of its challenges and resources.
  • Prior experience as an outreach worker or in customer service is a plus.
  • Knowledge of community resources and social service systems is beneficial.
  • Bilingual in Spanish is a plus.
  • Valid NJ Driver's License is required; NJ CHW Certificate is a plus.

  • Strong communication and interpersonal skills.
  • Cultural sensitivity and a passion for community health.

Responsibilities

  • Conduct direct in-person and telephonic patient outreach to engage with patients.
  • Document patient interactions and maintain accurate records of services provided.
  • Participate in meetings and contribute to program development efforts.
  • Collaborate with a multidisciplinary team to provide patient-centered care and support transitions of care.
  • Engage in community outreach to identify individuals who may benefit from health services and programs.
  • Meet patients in various settings to conduct screenings and set health goals collaboratively.
  • Make follow-up calls and home visits to motivate patients and connect them with community resources.
  • Maintain knowledge of community agencies and assist with patient advocacy and navigation.
