About The Position

The Community Health Assessment Coordinator implements and facilitates Maricopa County’s Community Health Needs Assessment (CHNA), Community Health Improvement Plan (CHIP), and the Maricopa Health Matters collective, which represents multisector community partners. The position exercises substantial discretion and independent judgment in designing and managing assessment and improvement planning processes, determining engagement strategies, interpreting and applying relevant public health guidance and planning standards, and recommending actions based on community health data and partner input. This work is done through active facilitation and collaboration of community and healthcare partners, data gathering and analysis, research, report writing, and presentation delivery for our public health department and healthcare partners. This position requires consistent collaboration with internal programs and community partners. A high level of responsiveness and accountability is expected through various forms of communication during the workday. Requirements

  • One year of supporting and coordinating public health community engagement programs or similar large-scale community-based project management requiring data collection and analysis, creating protocols, and monitoring deliverables
  • Bachelor’s degree in public health, community health, or a closely related field
  • A combination of post-secondary education and/or job-related experience may substitute for the minimum qualifications on a year-for-year basis
  • Proficient in Microsoft Word, Excel, and PowerPoint
  • Must possess or have the ability to obtain a valid Arizona driver’s license by the time of hire
  • Must have access to a vehicle to travel to and from weekly community meetings and events
  • Within one month of hire, all employees will be required to provide evidence of immunity or receive immunizations for vaccine-preventable illnesses for any position within the Maricopa County Department of Public Health; required immunizations include MMR (measles, mumps, rubella), Tdap (whooping cough and diphtheria), Varicella (chickenpox), and annual Influenza; some positions require a Hepatitis B vaccine and/or a Tuberculosis test

Responsibilities

  • Lead the development and execution of a community-driven Community Health Needs Assessment (CHNA), ensuring alignment with and documentation of MCDPH, PHAB, and MAPP 2.0 requirements
  • Design and manage comprehensive project plans, timelines, meeting calendars, task trackers, and deliverables lists; schedule meetings, prepare agendas, facilitate routine meetings, capture notes, and action items; prepare draft status reports, presentations, summaries, dashboards, and briefing materials for manager review; promptly escalates issues requiring policy, funding, or leadership decision
  • Initiate and maintain frequent interaction with community interest groups, academia, community-based organizations, healthcare, and public/private agencies exercising independent judgment in determining outreach approaches to obtain their active support and participation in the Community Health Needs Assessment (CHNA) and Community Health Improvement Plan (CHIP), in addition to other community health initiatives that address health for all, social determinants of health, and underserved populations
  • Support the Community Health Development team by planning, developing, implementing, leading, and evaluating the work of the Maricopa Health Matters (MHM) Collective through bi-annual MHM meetings, scheduling, communication, and strategic plan-aligned activities
  • Leads the development of the Community Health Needs Assessment (CHNA) through process coordination, presentation development, results sharing, and report writing
  • Collaborate Strategically with the communication team to develop training and informational materials to promote public understanding of MHM, the CHNA, and the CHIP to increase active participation and understanding; develop drafts, coordinate review, update materials, and manage dissemination and version control
  • Determine and implement evidence-informed outreach and data collection strategies in coordination with internal divisions and external partners, evaluate effectiveness, and adjust approaches to improve participation and data quality
  • Exercise independent judgment in managing data collection outreach logistics, including ordering, storing, and tracking supplies; training internal staff and external volunteers in data collection; and completing event registration, staffing, and follow-up
  • Support budget tracking and provide input on projected program needs, monitor spending against assigned line items, gather quotes, and track printing/translation/material costs
