About The Position

Under the direction of the Population Health Manager, the Community Health and Outreach Worker will assist in community outreach to ensure patients are scheduled for Covid vaccines, HEDIS measurement appointments, and appointments required by the MCOs.

Requirements

• High school diploma required.
• Community Health Worker certification or clinical medical assistant certification preferred.
• Experience in Community outreach preferred.
• Flexibility required with onsite and remote work.
• Must have strong oral and written communication ability necessary to communicate with agency staff and outside officials.

Responsibilities

• Contacts and schedules patients for appointments related to Covid vaccines, HEDIS measures, and MCO-related appointments.
• Builds working relationships and collaborates with outside organizations to achieve common goals.
• Assists with closing gaps in care.
• Assists with emergency room and hospital follow up appointments.
• Provides program information and patient education resources as needed to patients.
• Assists in management of referrals and closing referrals.
• Follows up by phone calls, letters or visit on missed appointments or referrals.
• Links patients with case managers of MCOs to assist patients with any case management needs.
• Performs related duties as assigned.
